Unlimited visits for new and chronic problems

Choice of in-person, telephone, or video visits, as appropriate
In-office procedures and tests -urinalysis, strep tests, ECGs, ear wax removal, minor medical skin procedures, albuterol treatments, etc

There is no long-term contract.
You may end your membership at any time by giving a 30 day written notice without penalty.
If you end your membership, and then want to rejoin later, the registration fee and current prices will apply.

Do you take health insurance?
No. Direct Primary Care practices do not take insurance. All visits and procedures that I can do in my office are included in the DPC monthly membership fee.
Can I still use my health insurance to pay for medications, labs, and x-rays that are ordered by Dr. Abousleman?
Yes, regular and PPO plans allow this. However, HMO and MCO type of insurance plans may not. Check with your insurance company to see if they will allow an out-of-network doctor to order these for you. Most HMO, MCO, and Advantage plans require covered care to be ordered by a provider who is in-network. You may want to consider choosing a PPO type of plan when you have a choice between PPO and HMO plans. Standard Medicare and Medicaid allow me to order and refer for you, but managed Medicare and Medicaid through a private insurer may not.
Should I cancel my health insurance plan once I start a DPC membership with Dr. Abousleman?
No. I recommend having health insurance or a health share plan to cover emergency, hospital and catastrophic care. If you see specialists more than once per year, it is also helpful to have insurance to cover the costs for visits and procedures they do. A DPC membership only includes care in my office.
